I was playing a game with 4 other people (we were a group of 5 premades) and we all know each othe irl. I had to afk for a reason and my friends were okay with it. I know for sure that they didn't report me, but do i still get banned if i afk too many times even if nobody reports me?

Yes, the Leaver Buster system is automated and does not require anyone to report you. AFK reports are mostly to catch people that are not participating in the match but are doing things that prevent the system from noticing.
Yes, you will eventually be punished for it; LeaverBuster does not require reports. Detecting an AFK player is super easy for an automated system. If you're not moving for too long, BAM, LeaverBuster.
That said, there is a bit of leniency to the system. Life happens, and having to run off due to an emergency once a month or so should not get you punished. Just don't make a habit of it and you'll be fine.
