Does a ban penalty last forever
do the ban penaltys go away after some sort of time
like if i get a 2 week ban then waited 1 year would i get another 2 week ban or a perma ban

It doesn't diminish over time, but by playing games and showing that your behavior is no longer negative (that you are reformed). If you just wait a year your account will be at the same bad status as it was when you left, and it won't take much to push yourself over the edge to the next punishment if you continue showing negative behavior. It is said that on average it takes 3 months of good behavior while playing regularly to decay 1 punishment tier.
It only goes down by playing. If you got a 14 day ban today and logged back in for the first time in 10 years, you're still on the chopping block for a perma

I know you've been helped enough but I'll share just to give you a little more comfort on it from hearing about someone else's personal experience. I earned my chat restrictions, 1 week(or whatever the first ban is), couple more chat restrictions and 2 week ban ban all within about 2 months. Sucks because I even have 4 rewarded summoner icons all from positive play through my years(not including the within 10% honorable oponent icon). All of the people flaming me just got to me and I would continue to argue back...
Anyways, After I got back from my 2 week ban I just shut it up for a couple of months, when the new honor system came out got countless most honored matches leading up to honor at least 50% to lvl 4 from locked within 2 months or more and then went on a spree of 3 chat restrictions/honor locks within 2 months and I've been clean ever since. They are very fair on giving chances and if you take them you will be just fine.
