How do I report someone that got boosted? I think its obvious that he got boosted because his summoner spells recently switched (flash on d from flash on f) . However, it was only his last 3 games so I dont know if that is enough to make him guilty.

why don't you confront the person first tho and ask him why he's doing it, and if he is tell him he should stop before potentially ruining his entire account and tell him if he is, you are going to report him.
