Why someone that wishes cancers and sweares families doesnt get banned from 1 game? Why he deserves second and third chance?

As both DuskDaUmbreon and PH45 said, it's highly unlikely that they get a third+ chance without either extensive time and effort on their part (to de-escalate tiers) or a rare misapplication of a lighter punishment by the system. As for why:
Riot believes that even those who commit exceedingly harsh offenses have the ability to reform, and, consequently, unless you can manage to be so singularly horrible and detrimental to the community in a single game, you're at least going to get one last chance to reform.

That's not a rule you can impose, especially when your initial question comes from an opinionated standpoint; you believe that the aforementioned player shouldn't get another chance.
In most cases these kind of insults lead to an immediate 14-day ban, but occasionally they get away with a chat restriction first.
Everyone gets treated equally in terms of rules and bans, but people who break the rules more severely jump to harsher punishments. Occasionally toxic players might get away with their behavior for few games but if they continue the act they will get punished.
Because everyone can have a day that just goes catastrophically bad, so Riot allows a second chance.
For that, though, they don't really get a third chance. I mean technically they do, but it takes tens to hundreds of thousands of games to get back to that point, which means...Maybe a few people who got punished in League's first year have gotten that chance again?
so why people are banned for flame? since there is a mute button there shouldnt be a report for toxic
and just so u know hate speechers are given at least second chance... already tested
Not all flamers get banned thats the point... the system sucks the same way as the criminal system
