These boards are full of people whining about being banned for exactly that.

I think your issue is that you didn't report them.

You can do (almost) anything you want in a game, and, if nobody reports you, you won't be punished.

My theory lies between the report type.

You report for harassment means that you think it's not as serious, thus an offense will take longer to punish. However, if you report for hate speech, that might get you somewhere.

Of course, you have to report them for the system to review it in the first place.

I reported someone who told me to go kill myself because I truly accidentally locked in mid (I had to go answer the door) and didn't know someone called it before I entered the room. I apologized and offered to go to another lane, but they told me to go kill myself and proceeded to flame me for 20 minutes as an autolocker. When I was doing well they stopped. We won the game but I reported them and nothing happened.

Because IFS is a robot. It does not make it's own determinations. It follows the script. If some one creates a "new" insult, it takes time for the robot to learn.

Does he not deserve it?

Eh. Personally, words don't bother me. Chat bans may be deserved. I wouldn't go to banning from the game entirely.

or why does he deserve second chance?

No one is perfect. If we never give anybody the opportunity to change, then they surely won't.

Hmm logically, the only rational way to explain that is he just didn't get reported. Otherwise, a definite error on Riots part.
