@Riot...What do you want a player to do with this situation?
A mid Kassadin who went "ad" and fed 0/10/0 at around 16 minutes and refused to ff. Do you really expect a team to carry him? How much does anyone want to bet he won't be punished/banned?

So, you've provided very little information here other than that he's trying an off-meta thing and isn't doing well with it at all. So no, nobody will expect someone to be punished merely based on the two little bits of info you've given us alone.
Assuming you believe the player is intentionally ruining the game, mute him, report him, and try your best to find something fun to do in the game. Your report will help him be punished, although it may not be immediate.

He won't be punished, but if you don't coddle him and tell him how great of a player he is then you will be banned.
honestly there's not much you can do other than report and move on
if you don't think the reporting system will suffice by all means send in a support ticket there's not much the boards can do to help
