Add a way to put a reason for a dodge due to toxic players in the pick/ban phase

I just dodged a game because we had a player who was being a troll and toxic as ever right from the start.

Scenario: First he wanted to play something other then top because he got filled there, then he wanted to play Ahri top, then he said he would play ADC, so our ADC said "OK i'll do that" and went Xin, after that, this toxic player decided he was going to play ahri top (picked after the xin) and refused to play bottom.

I was not going to sit through a game of toxic crud. Now I get to wait the 5 minute dodge because i wanted to avoid a toxic player while he gets to get off scot free. Please please add some kind of reporting method for toxic players either in the pick/ban screen or in the dodge screen.
