I just got done with an ARAM where an MF was tilted and gave up, after I told her to just play and it will be over soon she told me to "Kill yourself you brain dead monkey". I see a lot of posts on the boards about saying that phrase, is it an automatic 14 day or permanent ban?
I'd hope you reported it. It can also be escalated into a 14 day ban immediately or permanent if there was a previous offense.

Yep that's punishable. Encouraging self-harm is very serious and Riot handles it very strictly.
If you forgot to report after the game you can always try submitting a support ticket about the player.
