2-Week ban for DC'ing 1 time.

So i logged in yesterday to see that because i dc'd shortly after the start of a match, because i wasn't able to reconnect, that i was banned for a full 2 weeks. If your internet crashed, leaverbuster would've kicked in. Not the troll punishment.

Maybe next time don't try and avoid punishment by staying connected and "AFK"ing even if you're still moving around.

If you think this is an error you'll need to contact Riot support and ask them why you were punished.

You don't get a 14-day ban for disconnecting from one game. Your reform card says it's for trolling, griefing, feeding, or trying to lose. Maybe you should avoid doing those things in the future.

The first death in the middle of the lane, quite early on; the second below the enemy tower. Probably after that, you disconnected.

Based on that, we cant fully judge what happened. Possible you maybe inted the 2nd death, or made it very clear you were rage quitting, making this justified. Or a mistake - both is possible. We cant judge this case with these limited information, so contact support.
