Saying you're reporting them in game and outside of game is the same thing, It is still a bannable offense. So not you're saying to break the rules to save yourself from a punishment.
The system can't eventually pick up trolling since you don't have to actually feed to troll, and losses won't get you reported.
Where's your proof that this is the case? From my experience on the PBM forums and the PB and TBI forums of old the majority of punishable cases are games where the defendants team actually lost. As for feeding the troll trolls will troll regardless of what anyone says, hence why you report them for trolling.
If you don't get reported you can't get banned.
Generally this is the case however every now and then Riot will manually review certain cases and issue bans based on that. The thread I linked to earlier has an example of such a thing happening.
And the harassment I experienced was by far the worst and should classify for an instaban.
Everyone experiences harassment in-game from time to time, even forum regulars. Even if the harassment you experienced was the worst you've faced there's no way to know for sure if it was the worst out of everyone who plays the game. From Riot Tantram:

What many people fail to take into account is the perception of toxicity vs actual levels of toxicity and negativity bias. There are 9 other people in a game with you, and it only takes a single person having an off game to ruin your experience. That game, which is completely out of character for them, will likely not punish them. We understand that people have off games and a competitive experience can cause emotions to run high so we aren't going to ban a player for a a single off game (except in extreme cases).
We do however, rapidly (within minutes) punish people who make a habit of unsportsmanlike behavior. As you can see by browsing this section of the boards, people are punished all the time and the system is very accurate. The majority of players who receive a single punishment realize their behavior is not acceptable by the community and never receive another.
Nice try with your circular logic man, easy to defend a corrupted system when it works in your favor. Remember to spend some more money in the RP shop and get yourself a nice and shiny Elementalist Lux skin.
The system works fine from what I've seen over the years. While it is true that I have never received a punishment I was a fairly active participant in the Tribunal of old and on the player behavior forums of old as well.
By the way this board is useless since it works like 99% of reddit. Downvote what you don't like, delete everything else you don't like.
Generally the posts and threads in PB&M that are downvoted are just posts that are:
A) Whining and complaining.
B) Verbally abusive/harassment.
C) Low effort and intentional misinformation (i.e. lies).
D) Name and shame.
Posts and threads that are generally deleted here are:
A) Name and shame.
B) Verbally abusive/harassment.
C) Ones that break any of the other forum rules.
Some posts generally won't be downvoted if they put an effort into them and don't mindlessly attack anyone who disagrees with them.
