I never left a game so I am not going to agree to this. I need this leaverbuster warning removed.

Submit a support ticket, ask nicely for them to look into it. I'm sure that if this was a bug they will revert it and fix it for you.
Don't show up making angry demands though, that usually goes poorly for people submitting support tickets.
Its the automated thing that comes up the first time anything happens. Its not designed super well imo, but even if you D/C and reconnect instantly this pops up. Pissed me off when it happened to me as well. I know its not there for us, but its infuriating seeing it pop up the one time because comshit decided to fuck up.
At any time, did you AFK for 3 minutes in a game? I had that happen when I was making dinner, but I moved in the fountain for 2 minutes.
well, as long as you don't plan on leaving any games in the future, typing I Agree shouldn't bother you.
