There are a few things I noticed with the game in question:

  1. You're building Diana to be an attack speed duelist. While that's certainly not unheard of and is a decent build in its own right, you should take a look at the enemy team and see what you really should be building. The enemy team's carries consist of Syndra, Master Yi, and Varus. The AS duelist build would likely work against the Nunu, but both Master Yi and Varus will be able to kill you much faster than you can kill them with this build. I recommend building Diana like an assassin (Luden's Echo, Hextech Protobelt, Lich Bane, etc) to better deal with squishy carries when there's not much in the form of peel. I did a little bit of looking and your 13/9 Diana game 2 games earlier seems like a pretty solid assassin build.

  2. You're building entirely MR against a team that has both a Master Yi and a Varus. Your MR build would work against the Syndra and Nunu, but you probably found yourself getting easily bursted by Yi and Varus. Not to mention, the Varus has 32 kills, so he's where the largest amount of damage will be coming from. For this, I strongly recommend Ninja Tabi, Zhonya's Hourglass, or both).

  3. You have 19 cs at 10 minutes. 19. I realize that Diana is a very poor matchup into Syndra, but you should just be farming with your Q pre-6 and letting her push you into tower. In a bad matchup like that, you should aim for at least 50 cs by 10 minutes on a melee champion. (I realize the Jinx had a whopping 14 cs, but you can't fix anything about her gameplay so I'll just let it go)

  4. A total of 3 control wards were bought throughout the entire game, across both teams. You personally should try to aim for 3-4 control wards EACH. They're pretty cheap, and vision denial wins games.

Instant things that jumped out me.

Pick 1/2 roles. Pick 2-3 champions for each role. Play them exclusively. (When you've mastered a champion it's easier to focus on something other then their kit/items.) Cs better. (more gold = more opportunity to get objectives.) Buy control wards for vision and vision denial of objectives. (more vision = easier to make the right decision. (vision denial makes your opponents make worse decisions.)) Pick better fights or play safer. (With better champion mastery this will become easier.)

Work on those for a week and you'll see a large improvement.

See those champs with under 45% winrate? Don't play them. Play more Yi/Annie. They are both gods in low elo. Focus on farming more with Yi and dying less. Focus more on farming with Annie and don't die as much. Ward more. You have a bad vision score. Don't play Warwick anymore. You have a terrible win ratio with him. You're building Diana as a bruiser. Attack speed and ap. She is burst. Don't try to change that too much.

You have an insanely high amount of average deaths. I have 2.7 deaths average on my main right now, while you have 7-9 on your most played champions. This is a very typical bronze thing in my experience, so my suggestion would be to watch your replays and look at every time you died. Try and find out why you die so much, and from what mistakes it typically comes from.
