So i had to leave mid game
Due to unforeseeable circumstances i had to leave mid game.
What sort of penalties will be incurred?
Will this affect a permanent record?
Is there any way to make up for leaving or apologizing to the other players?

If you do it often, you get punished. Once a month or rarely because of real life stuff means you face less punishment.

About the permanent record, yes they do track that however if you play A LOT of games your Leaverbuster tier drops back to normal and nor will past offenses taken into account for future tiers once you accrue more leniency.
Leaverbuster punishes based on the rate you leave games not just individual leaves. The system is automated so you would be in low priority queue already if you were going to be punished.
It will count against you. If it was a rare occurrence then there really isn't any reason to care about it. If it's not then you should start putting more consideration into when you can complete a game before you queue. I guess you could say that it is on your permanent record but 1 leave is not significant.
if you really wanted to, you could look up the players from that game to friend them and apologize
