A Reform Card is an E-mail, or from what I've seen recently an In-game Client Message popup. That appears to let you know what you did wrong to warrant a ban on your account. The E-mail is usually given within the day of the ban, whereas i would assume the in-game Client pops up the moment you log in.
It basically tells you the reasoning as to why you were reported in your games and ban for that reason. It also shows at the very least the last game that triggers the ban to actually process. As such it might not have been that bad, but remember that it takes numerous amounts of reports from several games for you to actually get reported in which can result in a ban!
