Haven't played in two years, can someone catch me up?

Title says it all, I guess. Trying to get back into League after two+ years away. The last time I played, I think Thresh was the newest champion. Who's been nerfed to death, and who's been buffed? Are any of these new items good? Are any of the old items bad? What's the deal with this new jungle? Has the meta changed considerably? Any info is greatly appreciated, thanks a bunch.
