Loss Prevention rules?

I'm so confused on how to get a loss prevented. Last game I was mid and my top Pantheon fed 5 kills, called us a garbage team and left the game. Yet, a while ago, when only one person on the enemy team was having lag issues, everyone got a loss prevented. Just wondering why this happens.

Loss prevention is manually enabled for everyone during times of widespread server issues. It has nothing to do with what might be happening in an individual game.
