1.) Punishments are not likely from just a single game, However many games if players spam (Verbal Abuse) or (Offensive Langauge) or (Negative Attitude) you will get punished even if you were not toxic.
2.) Chat logs are reviewed, sure But the system is automated from what I have seen and experienced only chat logs with things like excessive (Harassment) are often audited and reviewed things like (End your Life Comments) (Threats) (Racism)
3.) People who report for no reason, good luck with that all the toxic match-up's I get players who spam report after about a week of having toxic players and I mean a lot of them if I play all day you get chat restrictions within the week some time.
4.) The number of restricted games is based off number of reports received if I get very few reports its like 12 games restriction the most I have gotten hit with is 38, if players spam assisting enemy team ,or Intentionally feeding, reports will lead to an automated ranked restriction.
^ The exception being if I play 1-2 games a day don't get reported as much just do a lot of aram, or normal games, then often I can go months without getting a restriction at least just one warning per month maybe.
Since Riot is so outstanding of wanting to improve players behavior, I can only hope that with the new Tribunal they will have a system that will show players themselves what games they were reported in with text showing their own toxicity and why they were (Chat Restricted) and so on, as well as offer a (Manual review) from player behavior specialists which can manually review a persons games which could take months and point out where the player was toxic or remove false bans by the automated system.
