Help from a high elo player (High Gold and above)

Hey I'm Silver 5 at the moment. Climbed up from Bronze 4 this season. I've been having a string of bad games for a while in ranked, going on tilt and such, but I was wondering if someone would watch a replay of a recent game I played and give me some pointers. I feel like, at least in the first 20 minutes I was doing very well (as the game progressed I started making some crucial mistakes) but it wasn't enough to carry.
