If you get a warning its done by an automated system which does often make mistakes.
. If you use certain context like "Die" a lot, or anything not toxic or hateful directed towards another it can be automatically flagged and if reported numerous times by trolls for harassment, or verbal abuse you can get automated restrictions.
. If you get reported a lot for intentionally feeding, or trolling in ranked it can lead to automated ranked restrictions.
I am sure after today I will likely get more automated restrictions because of spam reports although got no warnings lately and hope not to just too many toxic players today harassing chat, trolling champion select, mostly players trolling champion select by means of non communication, and throwing games, a couple of games where players fed and blame me for their loss, and few games where my jungler just came to my lane and got the enemy fed then harassed chat.
Doesn't matter the fact that I muted everyone after I clearly saw they were going to keep being toxic in game and stopped talking completely you will still eventually get automated restrictions, the current problem still being that Riot is not fully enforcing the summoners code which can be found here.

. Players who harassment chat and spam reports are usually the real abusers /trolls in the game proven fact.
. Players who do not communicate don't get banned or punished including in champion select.
. Players who don't listen at champion select and intentionally cause games to get thrown no punishment
. Auto Locking no punishment
. Junglers who run down a lane with no communication and give the enemy free kills. (A big issue lately) 3 games today this has happened and ruined the entire game in some cases games were still wins but almost thrown hard because of it.
The current system favors toxicity in my view.
