Is it me or is there something wrong with the grade you get at the end of a game?

So I've been doing pretty well in ranked today. But strangely, I haven't gotten an S on any of my games like I used to, despite me playing about as well as I used to. One game, I got 11/5/11 and another teammate got 12/5 or something along those lines, and neither of us got a S. Yet, in the same game, another teammate went 6/5/14 and got a S-. Later, I went 10/5 and got a B+, but in the next game I went 5/9 and got a B+ again.

How exactly is this happening and how are these grades decided? Is anyone else experiencing stuff like this?

The grading is based upon several factors including the champion you pick and the role you're in (this decides what metrics it holds most important I assume). It also compares your performance to the rest of the people who have played that same champion as that same role. From my understanding you need to be within approximately the top 2% to get an S rank (no idea if it's a solid S or an S-). The metrics used for determining the grade are unknown, but they are well past KDA for the most part and I think it's been agreed that CS and warding is a deciding factor. Other deciding factors could potentially be damage taken, damage dealt to objectives, and maybe even pink wards bought/wards destroyed (these are all my own theories and may not hold true).

A lot of people have been wondering the same thing but most simply post KDAs to prove their performance. Nothing against you or anyone who's done that, but I think we've become blind to thinking that KDA = performance when that simply isn't the case considering how much KDA is limited to.

AFAIK Grade are based on overall comunity performance .....

Getting S on lucian, for example, it's quite hard, since he is used a lot by Plat/Diam/Champ who make it harder to archive S score

Checked your match history. You didn't upgrade your trinket, and your cs is a little on the low side, same for your ADC in the game you were referring to. If you want S rank for mastery, you don't want to play Ranked. Because to get S rank, You want to try to average 8+ cs per minute (as an ADC) as I know of.
