One of my recent games had me trying to lead to victory, and the game-ending play went something like this.

Yorick (me) : Alright, I got my ult up and going, just focus whoever I mark with my E, OK?

Sion : CHAARRGE!

Ezreal Soraka & Taliyah all follow Sion's lead.

Sion proceeds to completely miss his ult, followed by the Ezreal missing his, the Soraka panic-ults, followed by Taliyah using hers and walls off my Maiden from me, leaving me to be picked off at the enemy team's leisure.

Off-topic, but I presume they rescued the Duckies too, right?
