This shit happens to the ADC as a support all the time...
Me: Danger Ping Danger Ping "Going B" ADC pushes all the way up and dies in a 1v3 as I'm walking back to lane ADC: "Report this shit support" Me: "..."
i had a jungle match the other day with the most idiodic player i have ever played with before in memory. i ganked for him, burned his opponents flash and figured "ok, that's deffinitely worth" so i backed off and went back to jungling. after my first back and a couple camps, i came and ganked again just as he had gone in on our mid laner, forcing him to ult away (and he had already burned his ignite on ziggs) so i figured it was worth again so i backed off. not 1 minute later this ziggs is complaining about how i never gank calling me a garbage jungler, telling me to uninstall, etc. i gank again a couple minutes later, force their ekko to burn ult and flash AGAIN and go to give ziggs my blue. a couple minutes after i give him blue he starts complaining again about how useless i am. i swear, had that not been a ranked game i would have just said screw the game, i'm going to make sure this guy gets destroyed. i wouldnt have even cared if i got reported i was so mad. i had made his lane 10x easier for him and he was acting like i was jacking off in base.
Sadly, I've got a friend that's the tank in this scenario. Just a few days ago, we were trying to win on ARAM... (we're part of a 5 man pre-made on voice chat, by the way)
Me - "Just let us know when you're planning on going in so we can follow up." Him - "OK" *10 seconds later he marks-and-dashes into the enemy team without warning* Me - "Crap! Why didn't you warn us you were going in?" Him - "Sorry, I guess I forgot." Me - "Well, just let us know next time" *Same thing happens again next time*
He's a great friend, but sometimes he thinks a little bit too much about the initiation and too little about the follow up :)

I used to be guilty of doing this sadly. Q_Q
But i have also had games where the most fed guy would say absolutely nothing the whole game, and i would not notice that this person was absent before a team fight and would end up following the rest of my team to our inevitable deaths. I dont usually get mad about this since i would blame myself for having poor map/chat awareness at the time.

Timing for a back is just as important as any other choice in league and shouldn't be done unless your in a good spot to do so...... if they are shoving and one of my team for some reason wants to back and by its like fare thee well my poor turret. Ideally lanes should be shoving and no important objectives should be on the table prior to a back.
