I once had an Azir blaming me for feeding the enemy jungler. I was still winning every fight with their Yasuo, but the bloody Shyvana got me every time..... He then told me the 0/5 Rammus was better than me. I'm pretty sure they were premade.

I've seen my friends do this rarely, usually in response to the other guy being toxic or just frustrating. I usually try to dissolve it.
Then the Alistair headbutts someone out of my knock up...at that point I wasn't sure if he was deliberately helping the enemy team or was just poor with timing (cause I'm poor with timing sometimes).

That happens to me too. I'm solo and the rest of my team was premade (I can tell because they all have similar names). I'm a support and they blame me for not carrying my adc while the top and mid also lose their lanes first. We lose that game and reported me for being such a bad support.
Yeah, this and boosting are definitely problems.
Then you're in a game with 3 friends and you have an absolute shitter on the team. Only you see it this time because you're not so biased at it being you.
Yeah I play with friends, get over it. If its your fault, I'll blame you. Can't speak for my friends though.
