There was a forum post way back in like 2011 that Nasus was inspired by a developers ex girlfriend who's name was Susan.
Hence the skill names

I think people mainly say "Susan" to be funny more than as a derogatory remark. The word itself mean nose or sense of smell which is somewhat fitting as he is a large bipedal, jackal headed ascended.
I main Nasus and never really notice if someone says Susan as I just re-translate it to Nasus in my head. I personally enjoy when the enemy team taunts me by calling me out as Susan in All Chat and then I do something like this to them...

I had a game not to long ago where I had 450+ stacks around the 20 minute mark, because no ganks top and the enemy laner kept pushing without being aggressive.
