booted at champ select randomly and given penalty

Can someone explain how it is even possible to have this happen? Our first banner went and when it was my turn to ban it immediately booted me and said I "did not make a selection". How does this bug even happen? How do you punish people for afking in draft when the system is broken and can punish people who aren't afk? Fix or remove penalty.
