[GAMEPLAY] Fiora perma ult

I was watching my brother play and noticed that the enemy Zac had a permanent Fiora ult around him. He would die, respawn, and still have the effect. Even worse, Fiora was unable to identify weakspots on any other champions. Don't know how exactly this happened and don't have any proof unless it be bad quality
