Hextech Box and Keys

I went into a normals game with a level 4 mastery Blitzcrank (me) and we won the game resulting in a level 5 mastery blitzcrank for me. However, when I went back to the hextech page, all I received was a key particle, not a hextech box I was supposed to get. What could have happened? Was that a glitch or am I doing something wrong?

I'm pretty sure you have to get a "S" rank on the champion mastery.

What exactly do I need to do to get an "S" rank though? Do I need a certain amount of cs or a high kill participation?
