Was stuck in reguler kayn for whole game. Ruined my level 7 progression.

I got my rhaast form first so i decided to wait for shadow assassin since we are badly in the lead, Would have been able get plenty more kills, but turns out i tried to click on the Shadow Assassin and it wouldn't work. So i decided to disconnect and retry but when i spawned in i lost all my progression and i could not gain any more souls and regular kayn model and rhaast model where combined and i was stuck as regular kayn with a glitched model. Got s- :(
