Double assignment to a certain role
Ok so this has happened 2 times, first 2 assigned to mid, then 2 assigned to adc now.

are you queuing with a 5-man party in which 2 or more members select the same role? that'll happen. Pretty sure if you're a 5-man premade you should be able to figure out who takes what role. :p
