[Gameplay] Character portrait bug?

I'm sure you guys have probably seen this bug a few times before, but I encounter it every now and then so I figured I'd report it.

Basically, when the bug occurs, either you or another player's champion portrait will be gone (replaced by a colored square and nothing else) or replaced by a picture of a summoner spell. Whether or not it affects their portrait on the minimap seems to vary. However, in kill/death announcements, in the ui, and on the scoreboard it will typically be bugged.

To be honest, I'm not 100% sure of the circumstances under which it happens, due to there not being anything outstandingly different about the games in which it occurs. If I had to take a shot in the dark, I would say it may be when you try to change your summoner spell at the last minute, when the game is launching, however that's just an educated guess,
