[Gameplay] Overcharge map-Teleported out of the map: unable to play till the end (gamebreaking?)

So. Today I played on game in overcharge mode. After ~9 minutes the score was 38 (my team) - 40. After my death I respawned & used middle(?) teleport from respawn &... I went here ---> https://imgur.com/a/uZ05N

I couldn't move till the end, so it was 2 vs 3 from that moment.

Watch closely my champion (Kog'Maw from 8:30).

I didn't played second overcharge game, because I 'm worried, that this bug will appear another time to me, or my teammates. But I think that this is enough to try to fix it.
