Alistar Ultimate

I was playing and was about to die and pressed his ult and died during the animation of him activating it. When i died there was no cooldown on it, but when i Ctrl+R it showed it was on cooldown but there was no indication on the icon and when i pressed it to use activate it there was no sound or message to say it was on cooldown or i could not use the ability at the time. Please check into this it is kind of frustrating and very deceiving. Thank you.
