Not sure if a one-off glitch or a bug in

Someone que-dodged during champion select and, naturally, my team recieved a notification saying this person dodged and that is the reason why we were placed back in the que. The problem that I'm not sure if is a one in a million glitch or a bug is that I was penalized for the randoms que dodge. My team was made to a wait a five minute period of time before we could re-enter the que. Thought this was something that I should report.
