Spamming teemo r nothing happened

It clearly showed that as omega-skinned teemo i had a shroom available and it wouldn't place it. Only when i had 2 shrooms would it place one.

Also another fun tidbit: I died when no minions were near me and no AA's were following me that i could see. Enemy donger didnt have the burn mastery. So not sure what happened there?
