Team Builder almost constantly fails to load skins. It happens if you play a game with a skin then play another Team Builder game again using the same champion and "Find Group" button without changing anything. After the first game the skin you had selected is ignored (even though it's highlighted) and the default skin is loaded. I know the skin is selected because I've just played a bunch of games on Shyvana. 1st game I had skin, 2nd and 3rd nothing. 4th I'm playing now I purposely went in to check the skin was picked and it was. I selected another, then the skin I wanted, game loaded and it was using my skin. Now, 5th game, I went straight into Team Builder and didn't change anything just hit "Find Group". Skin failed to load.

Easy to replicate. Pick a champion and skin in Team Builder, play as normal and then afterwards go back into Team Builder and hit "Find Group" without checking your skin it will always load the default skin even if you had (and still have when you check) a skin selected previously. I don't know why this has taken so long to find and fix but this is how you replicate it. Please fix it, it's very annoying.
