Rek Sai Tunnel Bug

Over 120 games on the underground beauty and only recently has this started happening, but there's been games where Rek Sai would go to make the tunnel and the tunnel would be created in the selected spot but she would be kicked out of the tunnel itself.

I've been playing rek'sai since the very first time she was released and I've always had this bug. I don't know how to replicate it and I've loose lot of games because of this. 3 years since rek'sai release and riot haven't fixed this malfunction.
