Graves Ult Bug

Its happened to me twice now, just then and quite a while ago, pretty sure it was in season 5 still. You die as you are firing your ult as Graves and the icon says it doesnt go on cooldown but you are unable to use your ult for the rest of the game Please, ive lost twice now because of this
