When reksai come out from ult and if target flash away from him. She deal 0 damage to enemy. Sorry for bad english. It means if flash away just reksai come out from her ult sje deal no dmg . And it happened everytime they flash away. Just in time.
Rek'Sai's ult is like a skillshot so can be dodged. This happens to me as well. Is not a bug.

It can be dodged, it's just hard to do. It is possible to dodge it just from having enough movement speed and running in a straight line, but you need a lot of movement speed to do that, so instead you can use a dash/blink to get away from her ult
that is intended she hops to the location the player was at when you hear the screech not where they are when it ends.still better then when multiple champs could avoid it by walking though.
