Here, I clearly have all 4 evolutions (hella satisfying), but the in game model does not show a single evolution other than the ult. Evolved Q and and then it disappeared after using the ability a couple times, same with every other evolution.

Thanks for the report! I'm not a huge Kha'Zix main, so forgive me if I'm not able to repro this in the same manner as you did. I evolved Kha'Zix fully, to check out attacking in each form, to see if anything disappeared. After attacking an enemy several times with each skill, and nothing disappeared.
I made a couple gifs to show you what I'm seeing:
If you have any additional repro steps, feel free to shoot 'em my way. Have you have this occur in every game you've played with him?

Unfortunately that was the last Kha'Zix game I've played recently, but heres the game.
I had 10% flat cdr, 9 armor and 14 attack damage as runes, and evolved Q into W into R. Having a Rengar on their team might be the factor. I played some games with Kha'Zix on the very same day and the bug did not happen, however, this happened when Rengar was on the enemy team.
