Here, I clearly have all 4 evolutions (hella satisfying), but the in game model does not show a single evolution other than the ult. Evolved Q and and then it disappeared after using the ability a couple times, same with every other evolution.

Thanks for the report! I'm not a huge Kha'Zix main, so forgive me if I'm not able to repro this in the same manner as you did. I evolved Kha'Zix fully, to check out attacking in each form, to see if anything disappeared. After attacking an enemy several times with each skill, and nothing disappeared.
