Client sends me to screen with "reconnect" button after champion select for all gamemodes [NA]

Happens with all gamemodes, even custom, practice tool, and tft. 100% of the time. This is also happening on my PBE account too. Tried reinstalling both normal and PBE clients and nothing worked. Repairing doesn't do anything. If it helps at all, the PBE client also has an "Error 004" when trying to update.

To clarify on the bug, everything is fine until after champion select. The client goes black for a few seconds and returns me to a screen where the only option is a "reconnect" button. Logging out and back in only brings me back to that screen.

I had this same exact issue with the previous patch, too. Me and a few other players reported it back then but nobody from riot responded. It quietly got fixed 3 days later. Anyone else having this problem? I haven't been able to play league since the patch dropped.
