Azir's Disc of the Sun

Even though I created a tower and the cooldown for his passive was still running, the icons to create towers were still on the map and locations of previous turrets. I attempted creating a turret, but the animation and graphics only happened. No turret was created.

This happened as soon as I died and was waiting to respawn. Unfortunately, I did not screenshot it.
