Defeat icon didn't appear

So recently i was in a game and when my team agreed to surrender the defeat icon didn't appear, so the only way for me to exit the game was by closing the program. Otherwise i would just be stuck there just looking at it forever.

I just had the same thing happen to me, except the victory screen never appeared. so i had to exit the game to be able to leave the stalled screen. But as far as my match history and MISSIONS are concerned the game never happened. What gives riot?
