LeBlanc Ult Not Activating?

I'm using Quick Cast with Indicators if that effects anything.

I've noticed that at times LeBlanc's ult either won't change to her current spell (issue 1) or won't cast at all (issue 2). At first I thought maybe the ult wasn't changing because of cast time, which would explain why I often jumped in by mistake when my ult is set on W. But during my recent matches I've used a Q and my ult was still on W. This happened multiple times. The ult also bugs out during the combo, either due to cast time or a bug. After using an entire combo I'll notice my ult is still up. I don't know. Does anyone else experience this problem?

I have the same issue too ever since the smartcast change. At first i thought it's just lag or packet loss making me see things because not many ppl have complained about it.

I was trying to figure out just what was the issue in a custom game. In the screenshot i had previously casted distortion so R was mimic distortion. And then I casted Q on enemy, trying to chain QR then mimic distortion indicator pops up. Then the indicator sticks around when I'm not holding down any key.
