GameBreaking Bug, Guarentees a loss when it happens

I have no idea why this happens, but it guarantees me a loss every single time and it seems to happen randomly

Could be a third party program. LolReplay (which you appear to have) has caused this in the past, though I can't say whether it's happening again.

You'll want to close all your other programs and run LoL by itself, then open your usual programs one at a time (restarting LoL each time if that's needed to trigger the crash) to find what's causing it. Anything with an overlay is particularly suspect. If you have to start your PC in Safe Mode you can do that. If the game crashes with nothing else open you will want to try repairing your client (you can try this first, doesn't take long) and/or updating your computer's drivers.
