Karthus Passive Bug

Every time i die with Karthus his passive activates now here is the real problem his Q has a non existing clone of it's self showing double just right off to the side once every time it is in use, so what i am seeing is when he dies his Q can take up the hole screen but the game isn't registering that when he dies in passive form Picture of Passive and Non Passive

Weird, are you using the default skin? can other people see it?

Well I just tested it in a custom game and it created two just like you said. Steps to recreate 1 Die with Karthus with the default skin 2 use his Q(lay waist) as far as it can go.
