Hold camera off-set for entirety of a match

I was playing an ARAM as Rek'Sai, and suddenly after I respawned once, my Hold Camera was permanently offset for only my champion. The team-mate lock on keys were still properly centered (F2 - F5). I took a couple screenshots so you can see exactly. My champion was a little up and to the right of the center of the camera. If you look at the minimap you can also see it's off center of my champion.

I think the biggest clue I have to go off of why this happened is when I died, I was being Urgot ulted. After I spawned the camera was messed up.
