Did not get a hextech chest - got S 1st time on champ, have chests available... PLEASE HELP!

This has kind of been happening on this account fairly often and I've finally captured another incident. I have 4 chests available, and just purchased Udyr - I have not acquired a chest this season with him. I just got an S in a game and received nothing.

Please help.!
