I was in aram last night and when I clicked on Malzahar's flash I got this, then clicked again and it was normal.

That is because lulu used poly-morph, when you are poly-morphed your character changes names, so whenever you type while poly-morphed or someone pings a poly-morphed champ the name changes, every skin of lulu's has a different name

Were you playing against a Lulu by chance? She has a nasty habit of changing your champ name sometimes with Polymorph. @_@
