Champion(s) Appearing Randomly

Can we address this, surely i'm not the only this has happened to. It even happened to Bjergsen in the semi's against CLG. A champion on your screen will appear there only to be half-way across the map. I don't know weather or not the game (your client) is telling you something else, but at the same time using the real-time info. Or the game is bugging because it thinks there are two of the champs. Whatever it may be, please fix this, It makes me use summoners only to realize that the graves "ganking" is top lane and not coming out of blue side bot bush.
