Random Disconnecting [CRASH]

Last game in Summoners Rift I consistently lost control of my champion (Tahm Kench) and was forced to leave the game then reconnect, one time the disconnect happened while reconnecting to the game. The only thing of note was that the enemy Cho'Gath was also disconnected for the whole game otherwise nothing was any different from usual, my ping wasn't high, and it happened at seemingly random points throughout the game, it also didn't affect my ping or bring up an "Attempting to Reconnect" message.

Will be continuing to play Tahm Kench and will let you know if I can figure out anything else.
